Section 16 of the regulations states that when a licensee becomes aware that a dose of radiation received by or committed to a person may have exceeded an applicable dose limit - I am leaving out some of the bits, if I may - then the licensee shall do the immediately do the following: notify the person and the commission; require the person to leave any work that is likely to add to the dose; conduct an investigation to determine the magnitude of the dose and establish the cause of the exposure; identify and take any action required to prevent the occurrence of a similar incident.
